I feel like we use it when we say, "wtf?!" Not a recommended phrase for every conversation, but definitely a great use of the interrobang (if you look it up, you'll see that early forms of it were made by typing a ? over a !--making me wish for a moment I had a manual typewriter!).

The rabbits have reminded me of a very bizarre conversation between me and my future husband on the day I met him. We were comparing notes on our medical conditions (a curious, unromantic - yet useful - topic for a first date), and he told me that the infusions he gets are ‘mostly mouse extract’!
Mouse extract! I love it! I love that you spent your first day talking about such things--so much better than the standard surface level questions. Thanks for sharing that! My brother will get a kick out of the mouse extract.
